As a field-based CNA, you’ll serve patients in their home or care setting by providing essential personal care services under the supervision of a registered nurse. You’ll work closely with families, support the care team, and help bring comfort and peace — one visit at a time. As a Certified Nursing Assistant (CNA) at Lifted Hospice, you play a crucial role in bringing dignity, comfort, and consistency to our patients and their families. You are often the most present member of the care team — the one who helps a patient feel seen, safe, and supported every day.

Requirements

  • Completion of a state-approved Certified Nursing Assistant (CNA) training program.
  • Active CNA certification in the State of Texas.
  • High school diploma or equivalent required.
  • Compassionate presence and strong communication skills.
  • Ability to read, write, and communicate clearly in English.
  • Comfortable using a smartphone for communication and documentation.
  • Valid Texas driver’s license, reliable transportation, and current auto liability insurance

Nice To Haves

  • At least one year of experience in hospice, home health, or a similar care setting is preferred.
  • Basic understanding of hospice care principles and a heart to serve.

Responsibilities

  • Following the Plan of Care – Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences.
  • Assisting with mobility and safety – Helping patients transfer, move, and ambulate safely — while maintaining a calm, clean, and secure environment.
  • Observing and reporting – Noticing and promptly reporting any changes in the patient’s condition or behavior to the RN, supporting proactive care.
  • Practicing infection control – Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
  • Respecting privacy and dignity – Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
