We are seeking a compassionate, reliable Hospice Certified Nursing Assistant (CNA) to support patients in the Reading, PA, and surrounding area. This role offers meaningful one-on-one care and a consistent weekday schedule, providing a true work-life balance. Requirements

Active Pennsylvania CNA certification
Valid driver's license and reliable transportation
Ability to pass a background check
Strong communication skills and compassionate approach
Ability to work independently and as part of the interdisciplinary team
Licensed or registered in the state in which currently practices
Must pass a criminal background check & MVR check.
Completed health statements acknowledging the ability to perform the duties of the position.
Valid state drivers license.
Proof of current automobile insurance.
TB testing per agency policy
High School Diploma or GED
Graduate from an approved training and competency course.

Nice To Haves

Hospice or home care experience preferred

Responsibilities

Provide personal care and comfort to hospice patients in homes and facilities
Assist with bathing, dressing, grooming, toileting, transfers, and mobility
Offer emotional support and companionship
Report changes in condition to the nursing team
Document services provided and follow the established plan of care
Support dignity, comfort, and quality of life at every visit
Maintains the agency's mission, philosophy, and core values.
Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
Always maintains patient confidentiality, including all HIPAA regulations.
