About The Position

We are seeking a compassionate, reliable Hospice Certified Nursing Assistant (CNA) to support patients in the Reading, PA, and surrounding area. This role offers meaningful one-on-one care and a consistent weekday schedule, providing a true work-life balance. Requirements

  • Active Pennsylvania CNA certification
  • Valid driver's license and reliable transportation
  • Ability to pass a background check
  • Strong communication skills and compassionate approach
  • Ability to work independently and as part of the interdisciplinary team
  • Licensed or registered in the state in which currently practices
  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • TB testing per agency policy
  • High School Diploma or GED
  • Graduate from an approved training and competency course.

Nice To Haves

  • Hospice or home care experience preferred

Responsibilities

  • Provide personal care and comfort to hospice patients in homes and facilities
  • Assist with bathing, dressing, grooming, toileting, transfers, and mobility
  • Offer emotional support and companionship
  • Report changes in condition to the nursing team
  • Document services provided and follow the established plan of care
  • Support dignity, comfort, and quality of life at every visit
  • Maintains the agency's mission, philosophy, and core values.
  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Always maintains patient confidentiality, including all HIPAA regulations.
