About The Position

By Your Side Hospice has grown out of the passion of an experienced team of hospice providers whose aim is to create a community-based, niche hospice to meet the unique needs of those we serve. We chose our name to reflect our core mission…to be by the side of our patients, families, and caregivers when they need us most. We seek a like-minded individual to join our team in the role of Certified Nursing Assistant (CNA). By Your Side Hospice is a locally owned company that provides hospice services in Milwaukee, Waukesha, Ozaukee, Washington, and Racine counties. Our work environment includes: Growth opportunities Flexible working hours Our organization is currently seeking a CNA to join our team and provide hospice and palliative care services to our patients. The successful candidate will be responsible for traveling to our patients and providing outstanding care based on the plan of care for each patient. This position requires a compassionate individual who is comfortable working with patients of all ages and backgrounds.

Requirements

  • Valid CNA license in State of WI
  • A valid driver's license & auto liability insurance
  • Use standard precautions to prevent the spread of infection and disease.
  • Knowledge of how to work a computer and smart phone
  • Understand English both written and spoken
  • Ability to work a flexible schedule, to include some evenings and weekends as needed

Responsibilities

  • Assists the patient in the Activities of Daily Living (ADL) including, oral & personal hygiene, baths, shampoo, skin and nail care, dressing, feeding, elimination, ambulation, reposition patient in bed or chair, and changing linens/making patient's bed.
  • Offers emotional support and encourages the home patient's family to participate in patient's care.
  • May assist nurse in giving treatments or assessing patients.
  • Uses DME equipment and supplies safely and properly. Maintain a safe and healthy environment.
  • Takes and records accurate patient vital signs or weight when advised.
  • Follows the Home Health Aide Care Plan as written, approved, and supervised by the Registered Nurse.
  • Observe patients' physical, mental, appetite and emotional conditions. Report abnormal changes or patterns to nursing staff. Informs RN changes in patient's condition.
  • Documents activities and findings and submits documentation as required by company procedures.
  • Provide Post Mortem Care
