About The Position

Now Offering a 5K Sign-On Bonus!!!! Primary Service Areas: Washtenaw County Schedule: Monday - Friday, 8am - 5pm, 40 hours a week No weekends or holidays! Hospice/Home Care Aide – Job Summary As a Hospice or Home Care Aide, you’ll provide essential support to clients in their homes, helping them with daily activities and ensuring they receive compassionate care. You’ll follow the care plan created by the nurse and physician, and work within the guidelines of the state’s Nurse Practice Act and other regulations. Supervision: This role does not supervise others.

Requirements

  • High school diploma or GED.
  • Completion of a federally approved training program and a competency evaluation.
  • At least one year of experience as an aide in a healthcare setting (home care, hospice, or institution).
  • Meets agency hiring standards, including professional references.
  • Able to work independently with minimal supervision.
  • Committed to ongoing learning and following agency policies.
  • Must demonstrate professionalism and ethical behavior.
  • Valid driver’s license and reliable transportation.
  • Frequent standing, walking, and lifting.
  • Occasional bending, twisting, and patient positioning.
  • Able to lift up to 35 pounds independently.
  • Comfortable working in a fast-paced environment with frequent changes.
  • Must be able to travel to various work locations.
  • You’ll help maintain a safe and respectful environment for clients, families, and staff.
  • All employees are expected to follow the organization’s Code of Ethics and Compliance Program in their daily work.

Responsibilities

  • Follow the care plan provided by the RN.
  • Report any changes in the client’s condition to the RN.
  • Help clients with personal care: bathing, grooming, dressing, mobility, and hygiene.
  • Assist with household tasks: meal prep, feeding, light cleaning, laundry, and bed-making.
  • Take and record vital signs as directed.
  • Support physical therapy exercises and range-of-motion activities.
  • Help clients with self-administered medications (under RN supervision).
  • Assist with safety equipment like wheelchairs and side rails.
  • Provide support with medical devices such as ostomy appliances or catheters.
  • Participate in care team meetings when appropriate.
  • Document services accurately and submit records promptly.
  • Maintain effective communication with clients and families.
  • Stay current with training and attend staff development programs.
