About The Position

The primary function of the Hospice CNA is to provide personal healthcare and related services to the patient/client in their place of residence. This includes assisting in providing a safe and clean environment, working cooperatively with the patient/client and family, and sharing observations and problems with the supervisor. The position works under the direction and supervision of a registered nurse.

Requirements

  • High School Diploma preferred.
  • Must have a current driver's license.
  • At least one year experience preferred as a Home Health Aide or Nursing Assistant in a hospital, nursing home or home health agency.
  • Must have successfully completed a State-established or other training and competency evaluation program that meets State and Federal requirements.
  • Must be able to read, write, and verbally report clinical information.
  • Proof of current CPR.
  • Successful completion of the Aide Competency Evaluation Skills Checklist and written Skills Test.
  • Reliable transportation and valid auto liability insurance.

Responsibilities

  • Provides direct patient/client care under the direction of the RN and according to the Aide Plan of Care.
  • Assists patients/clients with safe transfers and ambulation per Agency policy.
  • Reports changes and documents pertinent information and care rendered to ensure continuity of care.
  • Practices accepted infection control principles and provides a clean, safe, and comfortable environment.
  • Promotes positive, supportive, respectful communication to patients/clients, their families, and other employees.
  • Respects patients/clients and their privacy and property.
  • Assists patients/clients with proper nutrition and adequate fluid intake.
  • Contributes to the management and efficient operation of the Agency and demonstrates effective time management skills.
  • Performs treatments and procedures according to Agency policy.
  • Demonstrates commitment, professional growth, and competency by attending 12 hours of in-services per year.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
