About The Position

We are looking for a compassionate HHA or CNA to provide personalized care to patients in their homes during their end-of-life journey. The ideal candidate will assist with daily activities, personal care, and emotional support to ensure the comfort and dignity of patients.

Requirements

High school diploma or equivalent.
Massachusetts CNA or HHA Certificate is required.
Strong communication and interpersonal skills.

Responsibilities

Assist patients with personal care tasks, such as bathing, grooming, and dressing.
Maintains strict confidentiality of patient/family/caregiver information at all times
Maintain a clean and safe environment for patients.
Communicate effectively with healthcare team members and family caregivers.
Document care provided and patient progress accurately and timely.
