About The Position

Presbyterian Homes & Services – Optage Hospice is seeking an experienced RN or LICSW looking to use their knowledge and skills in a leadership position as a Hospice Clinical Supervisor. This position is ideal for those who feel called to hospice and wish to serve older adults within a caring Christian environment. This full time position operates Monday-Friday, daytime office hours, participates in supervisor on-call responsibilities with some flexibility to work remotely. Requirements

  • Current professional licensure as an RN, MSW, or LICSW in state(s) of practice and oversight.
  • Demonstrated clinical competencies in home care/hospice care management and operations, quality and business management, team building and employee development, client and family support systems, governmental regulations and community outreach.
  • Demonstrated knowledge and experience in hospice and palliative care or home care or clinical practices.
  • Must have valid driver’s license and proof of auto insurance.
  • Demonstrated computer proficiency, including Microsoft Office Suite and electronic medical records (EMR).
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.

Nice To Haves

  • Bachelor’s degree or higher, preferred.
  • Three to five (3 - 5) years of leadership experience in Home Care/Hospice, preferred.

Responsibilities

  • Serve as a leader providing supervision for a team of professional registered nurses.
  • Participate in the planning and implementation of strategies and programming that ensure high quality client care while achieving short and long-term operational goals and objectives for the Hospice program.
  • Provide primary oversight of referral intake and admissions coordination to ensure timely, accurate, and complete referral workups.
  • Balance intake workloads, ensuring appropriate staffing coverage and support.
  • Ensure documentation is completed accurately and within required timeframes.
  • Facilitate timely, clear communication across all stakeholders throughout the admissions process.
  • Ensure nurses are cross‑trained to support both visit and admissions responsibilities.
  • Oversee hospice transfers and re‑admissions, ensuring all required records are obtained, reviewed, and accurately entered into HCHB.
  • Coordinate and secure nurse practitioner or physician support for required face‑to-face visits related to re‑admissions.
  • Communicate re‑admission plans clearly to admissions staff and service coordinators.
  • Identify and deploy appropriate clinical resources for in‑person assessments as needed.
  • Ensure coroner pre‑registration processes are completed in accordance with policy when applicable.
