About The Position

Join our team and grow with us both professionally and personally! Acts is currently seeking qualified candidates for our Hospice Clinical Manager position. In this role, you will oversee and supervise the implementation of an interdisciplinary, individualized plan of care for each participant and family/caregiver unit, including assessment and management of pain and undesirable symptoms which affect comfort. You will also serve as a liaison between the Interdisciplinary Team (IDT), the Medical Director, the Hospice Clinical Director and Hospice Administrator regarding clinical problems and progress in care of hospice participants. The Hospice RN Care Manager will directly supervise hospice nursing personnel.

Requirements

  • Current state Registered Nurse (RN) license.
  • Minimum of 2-3 years experience in hospice or a transferable clinical venue such as home care, geriatrics, or oncology.
  • Current or eligible for certification in CPR.
  • On the job training of 3-6 months is required to learn hospice philosophy and policies & procedures.
  • Certification in hospice and palliative nursing within two years of employment is strongly encouraged.
