About The Position

The Per Diem Hospice CHHA provides personal care, comfort measures, and practical support to patients in their home or facility. This role is ideal for someone who is dependable, compassionate, and confident working independently. Experience working with patients on ventilators is preferred. Requirements

  • Active California CHHA certification in good standing.
  • Minimum 6 months–1 year caregiving experience (hospice or home health preferred).
  • Reliable transportation, valid CA driver’s license, and auto insurance.
  • Strong communication skills and a compassionate, patient-centered approach.
  • Ability to work independently in home settings.

Nice To Haves

  • Experience with ventilator-dependent patients preferred (training available).

Responsibilities

  • Provide personal care including bathing, grooming, oral care, dressing, toileting, and repositioning.
  • Assist with mobility, transfers, and safe ambulation.
  • Observe and report changes in the patient’s condition to the supervising RN or LVN.
  • Offer emotional support and a comforting presence to patients and families.
  • Provide support with household tasks directly related to patient care (e.g., changing linens, tidying patient care areas).
  • Assist with care for patients on ventilators , including comfort-focused positioning, equipment safety awareness, and reporting any issues to the nursing team (within CHHA scope of practice).
  • Document visits accurately and timely according to agency and regulatory guidelines.
  • Maintain patient privacy, dignity, and safety at all times.
