About The Position

The Hospi c e C haplain is responsible f o r the prov i sion of spir i tual care ser vi ces t o patient s an d fami l ies/caregiv e rs of Emblem Hospic e Tucso n , either directly o r through coordination o f c a re w i th other spir i tual coun s elors.

Requirements

  • Graduate of a ccredited s eminary or school of theology (BD, MD i v o r equi v alent theological degree). Exp e rience working w i th deat h an d dying, individuals/f a mi l y/car e giver.
  • Abil i ty to wo r k a s memb e r of interdisc i pl i nary group.
  • Comfort in an i nterfa i th setting.
  • Abil i ty to acce p t different lifesty l e s , culture s , beliefs, and values.
  • Abil i ty to networ k with community c l er g y and con g regat i ons.
  • Knowledge of and com m itment t o hospic e phi l osophy of car e .
  • If making home vis i t s , must b e a l icense d driver w i th an automobile that i s insured in accordance with s tate and/or organizatio n requirement s and is i n goo d w ork i ng order.

Responsibilities

  • Assures spiritual asses s ment of patie n ts and f a m i l i es/caregivers in the hospice program and appropr i ate s e rvi c e s ar e rendered in a t i mely manner.
  • Faci l itates th e development o f the individuali z ed the plan of care b y part i c i pating i n th e comprehens i ve asse s sment to meet ident i f i ed spiritual need s .
  • Provides dire c t sp i ritual c are to patient s and fami l ies/caregiv e rs.
  • Serves a s l iaison and support t o communit y chap l ains and spiritual cou n s elors.
  • Maintains recor d s of spir i tual car e s e rvi c es u til i zation and related activities for qual i ty assuranc e , p rogram de v elopment, an d pol i c i es and procedures review and revis i on.
  • Document s dire c t servi c es and ongoin g communication w i th c o mmunity c haplains and spiritual counselor s .
  • Attend s patient care conference an d interdiscipl i nary group meeting s a s a memb e r of the interdisc i pl i nary group.
  • Provides consult a t i on, e ducat i on, and support t o the interdisc i plinary gro u p on spir i tual care.
  • Recruits c o mmunit y chaplains, spiritual cou n selors, and sp i r i tual care vol u nteer s adequate to meet pat i ent and fami l y/caregiv e r need s b y de v eloping commun i ty contacts and offer i ng education through congregations abo u t hospic e c are.
  • Provides f o r funera l or memorial services for pat i ents a s requested.
  • Plans periodic memorial services to me e t the needs of personnel, volunteers, and commun i ty c l ergy/spirit u al counselors w o rking with th e hospi c e team .
  • Assists in supervis i on of spir i tual car e volunteers when assigned to p a t i ents/ fami l ies/caregiv e rs.
  • Actively partic i pates i n qual i ty asses s ment performance i mprovement teams and a c tivities.
  • Accepts an d perform s other related d u ties an d r e sponsibil i ties as required.
