About The Position

Hospice Chaplain- Join Our Innovative Team! Olive Grove Hospice Part Time/Full Time Pay Range: $25.00+ DOQ At Olive Grove, we believe hospice is about inspiring those we serve to fully live each moment. We are seeking a highly motivated, passionate Chaplain to join our team that will embrace our innovative approach and be able to work independently, take initiative and think creatively. This position does have the potential to grow into full-time with the right candidate. The Chaplain provides spiritual care and services based upon initial and ongoing assessments of the patient and family. Actively participates within the interdisciplinary team providing consultation and education to team members and families about spirituality and related care and services. Implements and coordinates the provision of spiritual care to alleviate distress and existential loneliness. Hours may vary depending upon census and program need.

Requirements

  • Ordained, commissioned or credentialed according to the practices of an organized religious group.
  • Comfortable in an interfaith setting.
  • Able to accept different lifestyles, cultures, beliefs and values.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.
  • OR Has a degree in theology or from a seminary.

Nice To Haves

  • Hospice experience preferred.

Responsibilities

  • Conduct a spiritual assessment as part of the comprehensive assessment within 5 days of admission to the hospice program.
  • Assist in development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care.
  • Provide spiritual support in accordance with on-going wishes and needs of the patient/family in accordance with their religious preferences or definition of spirituality.
  • Work in close collaboration with local clergy when desired by the patient.
  • Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
  • Evaluate the patients and family's response to, and effectiveness of, the spiritual counseling.
  • Participate in IDG and QAPI meetings and activities.
  • Conduct religious services of prayer, worship and rituals for patients/families, as appropriate.
  • Provide spiritual and bereavement support to staff as needed.
  • Provide consultation and education to the IDG on spiritual care.
  • Provide funeral or memorial services for patient’s if requested and as available.
