About The Position

Hospice Chaplain (Part-Time) Location: Nogales, AZ and surrounding areas Dependable Home Hospice and Palliative Care is seeking a compassionate Hospice Chaplain to join our interdisciplinary care team. Hospice care focuses on improving quality of life for individuals with life-limiting illnesses while providing meaningful support to their families and caregivers. The Hospice Chaplain provides spiritual care, emotional support, and bereavement counseling to patients and families while collaborating closely with our clinical team to ensure holistic, patient-centered care.

Requirements

• Experience as a Chaplain or in hospice/palliative care preferred.
• Training or experience in grief and bereavement counseling .
• Ability to provide compassionate support to individuals of diverse spiritual or religious backgrounds .

Nice To Haves

• Spanish-speaking ability is a plus , given the community served.

Responsibilities

• Conduct initial and comprehensive spiritual assessments for hospice patients and families.
• Provide spiritual counseling, grief support, and end-of-life spiritual care .
• Participate in the interdisciplinary team (IDG) to develop individualized plans of care.
• Ensure compliance regarding bereavement counseling.
• Coordinate with community clergy and spiritual counselors representing diverse faith traditions.
• Maintain accurate documentation of spiritual care services and related communications.
• Attend patient care conferences and IDG meetings .
• Provide education and support to staff regarding spiritual care and grief support.
