About The Position

Hospice Chaplain (PRN) Location: Phoenix, AZ and surrounding areas Dependable Home Hospice and Palliative Care is seeking a compassionate and flexible PRN Hospice Chaplain to join our interdisciplinary care team in the Phoenix area. Hospice care focuses on enhancing quality of life for individuals with life-limiting illnesses while providing meaningful support to their families and caregivers. As a Chaplain, you will play a vital role in delivering spiritual care, emotional support, and bereavement services to those we serve.

Requirements

• Experience as a Chaplain or in hospice/palliative care (preferred)
• Training or experience in grief and bereavement counseling
• Ability to support individuals from diverse spiritual and cultural backgrounds
• Strong interpersonal, communication, and listening skills

Nice To Haves

• Spanish-speaking ability is a plus

Responsibilities

• Conduct initial and ongoing spiritual assessments for patients and families
• Provide spiritual counseling, grief support, and end-of-life care
• Participate in the Interdisciplinary Group (IDG) to develop individualized care plans
• Support and ensure compliance with bereavement care requirements
• Coordinate with community clergy and spiritual leaders of diverse faiths
• Maintain accurate and timely documentation of all spiritual care services
• Attend patient care conferences and IDG meetings as needed
• Provide guidance and education to staff on spiritual care and grief support
