About The Position

The Hospice Chaplain provides spiritual care and counseling for patients and their family members in a caring, safe, and efficient manner in accordance with the individual's plan of care. This role supports the organization's mission and vision by completing an assessment of the patient's and family's spiritual needs at the time of admission and throughout the course of care. The chaplain coordinates with the patient/family and interdisciplinary team to establish, review, and revise the plan of care for each patient, providing individual spiritual care as needed or requested. The chaplain administers and coordinates the administration of sacraments, demonstrates an ability to minister to people with diverse faith traditions, and encourages employees to participate in the spiritual life of the location. The role also involves educating healthcare groups on spiritual and religious care, networking with community clergy, and addressing spiritual care referrals in a timely manner.

Requirements

  • Graduate of an accredited School of Theology with a Master of Divinity or a Master of Theology degree strongly preferred.
  • Completion of two units of Clinical Pastoral Education or being certified as a chaplain preferred.
  • Previous and extensive pastoral experience preferred.
  • Training in hospice and grief work is preferred.
  • Must be an active member in good standing of a Christian denomination.
  • Required to be ordained, licensed, or commissioned to function in a religious vocation.
  • Must provide written verification of ordination, licensed or commissioned status prior to being hired.
  • Current valid driver's license and must meet all medical guidelines for Sanford Health Category III drivers.

Responsibilities

  • Provides spiritual care and counseling for patients and their family members.

  • Demonstrates respect for cultural and faith diversity.
  • Encourages and trains employees in providing spiritual care.
  • Completes documentation according to policies and procedures.
  • Educates healthcare groups on spiritual and religious care.
  • Networks with community clergy and faith leaders.
  • Addresses spiritual care referrals and consults in a timely manner.
  • Participates in staff meetings and may serve on call as assigned.
