Job Description: Hospice Certified Nursing Assistant Location: Eaton County Schedule: Monday–Friday, day shift Elara Caring , we understand that hospice care is about so much more than medical treatment—it’s about dignity, comfort, and connection. As a Hospice CNA you will play a central role in helping patients and families find peace and support at home during life’s most meaningful journey.
Requirements
- High School Diploma or GED
- 1+ year of experience as a Nursing Assistant or Hospice Aide
- Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR §484.36)
- Reliable transportation, valid driver’s license, and auto insurance (50% travel)
- Ability to lift 50–100 lbs and perform hands-on caregiving duties
Nice To Haves
- Hospice or home health experience preferred
- Preferred to live in the service area
Responsibilities
- Provide hands-on, compassionate care following each patient’s care plan
- Offer comfort, dignity, and emotional support during end-of-life care
- Assist patients and families in learning self-care skills
- Report updates to the RN Case Manager and care team
- Document care provided, changes in condition, and patient needs
- Help prevent unnecessary hospitalizations and support continuity of care
