Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care. Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner. Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life. Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments. Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care. Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts. Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse. Accurately document care provided in accordance with Care 360 Hospice policies and procedures. Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences. Provide support and education to family members and caregivers within the scope of the CNA role. Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance. All other duties as assigned
