Provide continuous bedside presence during a patient’s active transitioning phase, ensuring they are not alone and are supported with dignity, peace, and comfort in their final hours. Perform personal care services including bathing, dressing, oral care, incontinence care, and gentle repositioning to maintain comfort and skin integrity. Offer compassionate companionship, therapeutic presence, and emotional reassurance to patients throughout the dying process. Create and maintain a calm, quiet, and respectful environment that honors the sacred nature of end-of-life care. Provide respite and supportive presence for family members and caregivers when on-site, allowing them moments of rest while ensuring their loved one remains attended. Provide emotional support and guidance to family members during the active dying phase. Observe and promptly report changes in the patient’s physical, emotional, or behavioral condition to the Registered Nurse Case Manager. Report any deviations from the established plan of care to the Case Manager and interdisciplinary team Accurately document all care provided in accordance with Care 360 Hospice policies and regulatory standards. Safely and appropriately use durable medical equipment in accordance with agency guidelines and training. Perform postmortem care with dignity, respect, and adherence to hospice protocol. Perform additional duties as assigned to support quality end-of-life care. All other duties as assigned
