As a Hospice CNA, you will play a vital role in providing personal care, emotional support, and assistance with daily living activities to patients receiving end-of-life care in their homes or facilities. Schedule: PRN, Weekends, Weekdays, Monday - Friday Primary Responsibilities and Duties: Assist patients with personal hygiene, including bathing, grooming, and dressing. Monitor vital signs and report any changes to the supervising nurse. Administer medications and treatments as directed by the healthcare team. Provide companionship and emotional support to patients. Assist with mobility and positioning to enhance comfort and prevent bedsores while ensuring the patient's safety. Support families in understanding and coping with the emotional aspects of end-of-life care. Maintain a clean and safe environment for patients. Accurately document care provided and report any significant changes in the patient’s condition. This includes noting exact medication and care provided with the date and time for insurance purposes and appropriate quality of care to patients. Adhere to all safety protocols and infection control measures. Collaborate with family members and health care teams to ensure continuity of care. Adhere to infection control and safety procedures to protect patients and staff. Travel to patient homes and facilities within the Lehigh Valley and surrounding areas.
