About The Position

Perform personal care for the patient including bathing, dressing, oral care, and repositioning Document care provided according to the Care 360 Hospice policy. Report any deviations from the plan of care to the case manager. Report any changes in the patient's condition to the registered nurse case manager. Use durable medical equipment safely Provide respite for the patient's family/caregiver when on-site, as appropriate Sit alongside patients during their active departure. Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences. Provide support to the patient's family Perform postmortem care Other duties as assigned

Requirements

Licensed Nursing Assistant in state of practice (CNA or STNA)
CPR/BLS Certified
Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
Must be willing to travel
Valid Drivers License and Auto insurance Coverage with reliable transportation

Nice To Haves

Hospice experience preferred
