About The Position

We are seeking a Full-Time Hospice RN Case Manager to join our growing hospice team in the Cincinnati area. In this role, you will coordinate and provide skilled hospice care to patients in their homes, ensuring comfort, dignity, and quality of life during end-of-life care. Schedule: Full-Time, M-F, On Call Rotation At Advanced Home Health and Hospice of Cincinnati, we are committed to delivering compassionate, patient-centered care while building meaningful relationships with patients, families, and our community partners. Requirements

  • Active RN license in the State of Ohio (required).
  • Knowledge of hospice philosophy and end-of-life care principles.
  • Excellent clinical assessment, communication, and organizational skills.
  • Compassionate, patient-centered approach to care.
  • Valid driver’s license, reliable transportation, and current auto insurance.

Nice To Haves

  • 2 years of hospice experience preferred but not required

Responsibilities

  • Manage a caseload of hospice patients, providing skilled nursing assessments, care planning, and symptom management.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide education, emotional support, and resources to patients and families.
  • Communicate regularly with physicians, hospice aides, social workers, chaplains, and other team members to ensure comprehensive care.
  • Document all care provided accurately and timely in compliance with regulatory standards.
  • Participate in regular case conferences and team meetings.
  • Maintain flexibility to assist with weekend coverage as needed.
