About The Position

Hospice Care Coordinator role with MercyOne Home Care/Hospice Hospice Care Coordinator Location: Des Moines Position Purpose: The Hospice Care Coordinator plays a crucial role in facilitating referrals to hospice and post-acute home care services from the acute care setting. This position also involves educating healthcare staff, including physicians, on the services available from hospice. In addition, the Hospice Care Coordinator may participate in business development efforts related to expanding services or launching new programs. Key Responsibilities: Regulatory Compliance: Maintain a thorough knowledge of Federal, State, and local laws, coverage requirements, as well as organizational policies, including the Integrity Program and Code of Ethics. Ensure compliance and demonstrate honest, ethical behavior in all duties. Referral Management: Maintain daily communication with case managers, discharge planners, and hospital units to facilitate patient referrals for hospice or home care services, ensuring a timely and efficient follow-up process. Interdepartmental Collaboration: Work closely with healthcare team members to provide information on hospice and homecare services, answer questions, and assist with patient discharge planning from acute care settings. Minimum Qualifications: Education: Bachelor’s degree or equivalent level of experience required. Licensure: Graduate of an approved clinical education program and current clinical license in the state of employment is preferred. Experience: Minimum of two (2) years' experience in an acute care setting, customer service, sales/marketing role, or relevant experience. Other Requirements: Must possess a current Driver’s License and have reliable transportation to and from work sites. Requirements
