As the Patient Care Manager Assistant, you are responsible for assisting the Patient Care Manager in coordinating a team of staff to assure the continuity of high-quality care to patients assigned to the team's area. This is a Full Time position in Hospice. This position will include Hospital and Home Visits for hospice support and patient care coordination. This position will include direct patient care as needed. LPN certification in NC required. Our Business Hours are Mon-Fri, 8AM-4:30PM. Our agency covers a 50 mile radius. Candidates must be able to see patients within this area.
