About The Position

Under the supervision of the Clinical Manager or designee, the Hospice Care Aide performs basic patient care activities necessary in caring for the personal needs and comfort of terminally ill patients in various settings. As a part of the organization, designated hospice services, and the interdisciplinary team, hospice aides have the responsibility to contribute to the comfort and dignity for a patient and provide care that is consistent with patient and family needs and goals, with patient needs and goals as priority. The Hospice Care Aide provides services that are: • Ordered by the interdisciplinary group. • Included in the plan of care. • Permitted to be performed under State law by such Hospice Care Aide. • Consistent with the Hospice Care Aide training. The Hospice Care Aide works along with the interdisciplinary group (IDG) to provide quality patient care, weekend and holiday work is required.

Requirements

  • Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
  • Current BLS Certification.
  • Current AIDS 104 Certification.
  • Current State of Florida Driver's license with driving record acceptable to Community Hospice & Palliative Care insurance carrier.
  • Reliable transportation.

Nice To Haves

  • High school diploma or GED equivalent preferred.
  • Six (6) months experience in home care, hospital, and/or long term care.
  • Experience in caring for the terminally ill preferred.

Responsibilities

  • Perform hands-on personal care.
  • Perform simple procedures as an extension of therapy or nursing services.
  • Provide assistance in ambulation or exercises.
  • Provide assistance in administering medications that are ordinarily self-administered.
  • Report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse.
  • Complete appropriate records in compliance with the organization's policies and procedures.
  • Teach and train family/caregiver proper techniques to meet the personal care needs of the patient.
  • Maintain patient confidentiality at all times.
  • Attend team meetings and participate in other agency meetings as directed.
  • Maintain a minimum of twelve (12) hours of training/education specific to position annually.
  • Other duties as assigned.
