Responsible for planning, implementing, and maintaining a bereavement program that supports the families/caregivers of the deceased for up to one (1) year after the patient's departure. Development and implementation of the bereavement plan of care for the bereaved following the hospice patient's death. Develop a bereavement plan of care using the bereavement assessment, the risk assessment, input from the IDG, and interaction with the bereaved following the patient's departure. Provide bereavement support to hospice personnel coping with work-related grief. Provide memorial services, grief support groups, and community education, send mailings, bereavement calls, bereavement visits, and other services deemed necessary.
