Position Overview: The Bereavement Coordinator completes Bereavement Assessments, establishes Bereavement Plans of Care and provides/arranges Bereavement contacts according to the Plan of Care. Territory: Madison and Limestone AL Schedule: Monday through Friday Essential Job Functions: Complete Bereavement Assessment as per policy timeline. Ensure initial bereavement contact made by staff (attend wake and/or funeral) Establish Bereavement Plan of Care appropriate to needs of family members identified in Bereavement Assessment. Coordinator/schedule bereavement contacts per Bereavement Plan of Care and Assessment: visits, phone calls, letters, cards, bereavement material/literature, memorial services, and other bereavement activities. Establish Bereavement chart Document all Bereavement contacts and file all contacts in Bereavement chart Maintain, update, and review Bereavement Chart for compliance with Bereavement Plan of Care. Continue to monitor family member bereavement needs and coping skills. Determine if need exist for outside grief counseling or other resources. Why Join Our Team?
