About The Position
Position Summary: The goal of hospice and palliative nurse RN Case Manager is to promote the patient's quality of life through the relief of suffering along the illness trajectory, through the death of the patient and into the bereavement period of the family. This goal is enhanced by a competent level of practice as demonstrated by the Standards of Hospice and Palliative Care Nursing Practice. Requirements
- Minimum of one year in an acute care setting or comparable work experience required.
Nice To Haves
- Minimum of one year oncology and home care experience preferred.
- Minimum of one year Hospice experience preferred.
