The Hospice & Palliative Care QAPI (Quality Assurance Performance Improvement) Specialist will work at the direction of the Director of Quality, collaboratively with the Directors of Hospice and Palliative Care, appropriate Managers, members of the QAPI Committee to support Hospice and Palliative Care QAPI activities and to meet performance improvement objectives. They are responsible for developing, implementing, maintaining, and evaluating the QAPI Program for Palliative Care, and collaborating on developing, implementing, maintaining, and evaluating the QAPI Program for Hospice. This includes monitoring patient clinical records and providing relevant staff education and coaching. Leads and evaluates performance improvement processes using best practices and established protocols to effect change. Coordinates data collection and reporting needs in accordance with clinical standards and expectations, agency policies and procedures, and applicable laws and regulations. Establishes systems to ensure the integrity of data input and reporting. Schedule: Monday - Friday; 8:30am - 5:00pm Location: 4 days onsite; 1 day remote Based in Larkspur, CA with occasional travel to our Pleasanton, CA and San Mateo, CA Offices By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
