Job Summary Provides personal care, exercises and meals to assigned patients. Documents care given. Receives supervisory direction from the professional nurse or therapist regarding patient care assignments. Transports medications and requests to and from Pharmacy/Nurse on a non-routine basis when necessary. Delivering world-class health care one patient at a time.

Requirements

• High school graduate or equivalent, knowledge of medical terminology.
• Minimum of one-year SNF/Acute care experience required.
• Excellent customer service skills required.
• Current valid Drivers License.
• Auto liability insurance required.
• Current CNA licensure.
• Certified by the State of California as a Home Health Aide (CHHA) within 6 months of hire/transfer.
• BLS required.
• Management of Assaultive Behavior (MAB) Training Certification that meets AB508 and SB1299 required within 30 days of hire/transfer.
• New hires who have completed MAB training outside of Emanate Health will be required to complete Emanate Health Work Place Violence online awareness module.

Responsibilities
