The Hospice Aide provides in-home services to patients, delivering care as ordered by a physician and indicated by the plan of care. This includes assisting patients with daily living activities such as baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing, feeding, and transferring. The aide is responsible for taking accurate vital signs, positioning patients, and assisting with ambulation. They must perform patient care in an environment that optimizes safety and reduces errors, while also supporting cultural practices that do not interfere with medical treatment. The role involves participating in case conferences (IDG’s) to discuss patient progress and care plans, and communicating findings and patient complaints to appropriate licensed staff, including the Case Manager, Clinical Manager, or supervising RN. Clear and concise documentation in the medical record is also required, and the aide works as part of an interdisciplinary group.
