About The Position

Summary of Essential Functions for the Position: Committed to Caring our mission, a Hospice Aide is specifically trained, competent and performs the provisions of hands-on personal care, the performance of simple procedures as an extension of therapy or nursing services, assistance in ambulation exercises, assistance in administering medications ordinarily self-administered (assistance is limited to getting water or fluids) to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). The Skilled professional staff whether direct employee of the agency or under arrangement are expected to contribute to all phases of the QAPI program and participates in with ongoing interdisciplinary assessments of patients. The Hospice Aide will participate in the development and revision of the plan of care in partnership with the patient, patient representative (if any) and caregiver(s).

Requirements

  • Meets one of the requirements of the Texas Administrative Code (state.tx.us); Chapter 558.
  • Certified Nursing Assistant (CNA) in the state(s) in which practicing, preferred.
  • CPR Certified with American Heart Association or American Red Cross required.
  • Ability to exercise initiative and independent judgment.
  • Strong communication skills, both oral and in writing, with excellent interpersonal skills.
  • Must demonstrate good customer relations skills and a commitment to providing quality service.
  • Acceptance and ability to demonstrate support the core values and goals of Agency.
  • Must have a current valid drivers license, auto liability insurance and reliable transportation.
  • 100% travel may be required.
  • High School diploma or GED is required.
  • Must have completed at least forty hours of approved training and competency course to meet the Medicare Conditions of Participation and State for Home Health Aide training program and competency is required.

Nice To Haves

  • Certified Nursing Assistant (CNA) in the state(s) in which practicing, preferred.
