About The Position
Hospice Aide Coordinator What You Must Have: High School Diploma Preferred If an aide, has successfully completed one of the following: Training program/competency evalas specified in §418.76(b)(c)of the Medicare regulationsCompetency eval program that meets §418.76(c) Nurse aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry State Licensure program that meets requirements of §418.76(b)(c) Licensure: CNA preferred At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.One year experience as an aide scheduler preferred. Responsibilities
- Utilize the EMR software system to schedule clinical staff patient visits
- Schedule patient visits according to care plans and staff availability
- Confer with Patient Care Manager and/or Clinical Supervisor on problems and complaints timely
- Verify documentation of information such as completion of visits, frequency & duration of visits
- Oversight and tracking of staff in regard to punctuality, assignments and reliability
- Timely submission of all documentation
- Performance of other duties as required
- Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
