About The Position

As a Hospice Aide, you are one of the most vital pieces of the interdisciplinary team, serving as the boots on the ground and the main communicator of your patient’s needs. The CNA's role is to follow a thought-out care plan for each patient and to be the right hand of the RN Case Manager, ensuring the best care for the patient is achieved.

Requirements

• Current CNA Certification in the State of Georgia
• Current CPR & First Aid Certification
• Reliable and legal transportation to and from patient's place of residence

Nice To Haves

• Hospice or home care experience is a plus
• Kindness and reliability

Responsibilities

• Follow a thought-out care plan for each patient
• Be the right hand of the RN Case Manager, ensuring the best care for the patient is achieved
• Be the main communicator of your patient’s needs
