About The Position

The Elizabeth Hospice, Hospice Aide is primarily responsible for providing direct personal care to terminal patients in their homes. Also responsible for participating in patient team updates. Responsible for carrying out the mission, vision, values, and quality commitment of The Elizabeth Hospice.

Requirements

  • Post high school Vocational/Specialized Training.
  • Minimum of 6 months experience (waived with special consideration of Team Manager).
  • Must possess current California CHHA and CNA Certification.
  • Current CA driver’s license, proof of insurance, and reliable transportation.
  • Computer skills (word processing), use of calculator, fax, and photocopier.

Nice To Haves

  • Preferred experience of 1-2 years direct patient care.
  • Spanish speaking preferred.

Responsibilities

  • Provides personal care to patients in their homes, SNFs, & RCFEs with consideration for age-related differences.
  • Meets their Activities of Daily Living needs as assigned by RN.
  • Coordinates personal care services, scheduling visits with patient/family.
  • Observes and communicates changes in the patient’s condition to the primary RN and/or Team Manager and staff at facilities as needed.
  • Attends discipline-specific and all-staff meetings and maintains continuing education credits for licensure.
  • Recognizes needs of patients/families and responds appropriately within the scope of practice.
  • Performs related tasks per plan of care which may include staffing crisis care, making deliveries, light housekeeping, and other tasks as assigned.
  • Documents observations accurately according to charting standards.
  • Maintains timely patient documentation using 'point of care' electronic charting.
  • Performs basic skills included in competencies checklist, policies/procedures and standards that are required for agency.
  • Reports to primary RN or Team Manager any changes of condition, injuries of unknown origin, or patient/family/facility concerns in a timely manner.
  • Adhering to the E-way standards is a requirement in performing all job duties.
