About The Position

Optimal Care is actively building its talent pipeline for future opportunities and invites interested candidates to submit their resumes. Optimal Care is a clinician-owned and operated company dedicated to fostering employee potential while maintaining a personalized focus on patients and families. As the Midwest's premier provider of Physician Services, Home Health, and Hospice Care, Optimal Care utilizes an integrated care delivery model incorporating technology, innovation, and best practices to achieve value-based outcomes in chronic disease management, rehabilitation, and end-of-life care. The company's mission is to Serve Together, Provide Value, and Deliver Exceptional Quality Care, committing to being an exceptional place to work where expertise, passion, and commitment to quality care can thrive. This after-hours Registered Nurse role offers a unique schedule of working on-call for 7 consecutive nights (5pm-8am, Monday-Sunday) followed by 7 days completely off, effectively working only 26 weeks per year for a full-time salary. This schedule provides ample personal and family time, with no additional on-call requirements and optimized territories for manageable drive times to meet urgent patient needs. The role is supported by a triage team, meaning after-hours RNs are dispatched only when an in-person visit is required, and they are not responsible for patient triaging, managing a case load, or ongoing management of hospice patients. Instead, they function as an "emergency room" nurse for hospice patients in the community.
