Schedule: Full-Time Job Overview: As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.
Requirements
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State license as a Registered Nurse; Current driver’s license. Current CPR Certification.
- Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice.
- Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations.
- Good interpersonal skills.
- Reliable transportation and valid auto liability insurance.
Responsibilities
- Coordinates and oversees all aspects of new patient admissions
- Collaborates with patient care teams to ensure seamless communication
- Educates families on patient condition, needs, and care
- Performs head-to-toe physical assessments
- Timely and accurately completes documentation
- Other duties as assigned
