The Hospice Admissions Registered Nurse (RN) provides specialized nursing services in accordance with the patient's plan of care. This role involves managing a caseload of approximately 12-18 patients, which can vary based on patient/family needs, geography, and whether care is home-based or facility-based. The RN demonstrates competency in a wide range of nursing skills, including pain management, symptom control, infusion therapy, wound care, and aseptic technique. A critical aspect of the position is applying critical thinking, analysis, and clinical judgment to care for patients and families facing terminal illness, addressing their physical, emotional, psychosocial, and spiritual needs. The RN coordinates the total Plan of Care, liaises with other health professionals, attends interdisciplinary group meetings, and initiates patient care conferences as needed. Patient and family education on their rights and responsibilities is also a key component. The Hospice Registered Nurse acts as a Case Manager, overseeing patient care from admission through discharge, ensuring equitable and beneficial resource utilization, and coordinating with the interdisciplinary team on bereavement.
