This position focuses on assessing needs, developing care plans, and providing education and emotional support at the start of their hospice journey. You’ll have the opportunity to provide one-on-one care —both in person and over the phone—ensuring that every patient and family receives compassionate, individualized attention. Hours: 4 10 hour days with Tuesday as the day off. Key Responsibilities Conduct comprehensive patient assessments and initiate hospice services Develop and coordinate individualized care plans Provide education, counseling, and emotional support to patients and families Collaborate with the interdisciplinary team to ensure seamless transitions of care Cover patient care boards and make home visits as needed across our service area Participate in daily team reports and ongoing communication with staff Why Join Our Team This is more than a nursing job—it’s a calling. Requirements
- Current New York State RN license
- Minimum 2 years of nursing experience preferred
- Experience as a Hospice Nurse , Home Health RN , or in palliative care, oncology, or acute care strongly preferred
