At Mountain Valley Hospice, admissions are more than paperwork — they are the first step in building trust with patients and families during one of the most important transitions of care. We are seeking a compassionate, organized, and clinically confident Registered Nurse to serve as an Admissions RN for skilled nursing facilities (SNFs) and assisted living facilities (ALFs) in the Forsyth County area. As an Admissions RN, you will be responsible for coordinating and completing hospice admissions in facility settings, ensuring a smooth, timely, and supportive transition for patients, families, and facility partners. This role is ideal for an RN who enjoys building relationships, values structure and follow-through, and finds purpose in guiding patients and families through the hospice admission process with clarity and compassion.
Requirements
- Current RN license in North Carolina (or compact state)
- Strong clinical assessment skills and attention to detail
- Excellent communication skills and comfort working with patients, families, and facility staff
- Ability to work independently while collaborating closely with a team
Nice To Haves
- Hospice or admissions experience preferred (but not required)
Responsibilities
- Completing hospice admissions in SNF and ALF settings
- Conducting thorough clinical assessments and eligibility evaluations
- Educating patients, families, and facility staff on hospice services and expectations
- Collaborating with referral sources, physicians, and the interdisciplinary team
- Ensuring accurate, timely documentation to support compliance and quality care
- Serving as a professional and compassionate representative of Mountain Valley Hospice in the community
