About The Position

The Admissions Registered Nurse (RN) at ANX Hospice Care is a vital member of the Interdisciplinary Team (IDT) responsible for presenting hospice services to patients and families, ensuring appropriate admissions. This role involves performing consultation and admission visits, assessing patient needs, and completing necessary documentation. The admissions nurse must understand hospice programs and benefits, evaluate patients for hospice appropriateness, document assessments, and initiate care plans. Responsibilities also include coordinating medications and DME needs, communicating with the medical team, and participating in on-call rotations. The position requires flexibility in scheduling and a commitment to high standards of care.

Requirements

  • Must be able to work in San Francisco & San Mateo County.
  • Current CA Registered Nurse (RN) license.
  • Current CPR card for Healthcare Providers.
  • 2 years of Registered Nurse (RN) experience in Hospice Care.
  • 1 year of Hospice Admissions Nurse experience.
  • Licensed driver with an insured automobile in good working order.
  • Understanding of hospice philosophy and experience with terminally ill patients.
  • Demonstrated nursing skills and ability to cope with family emotional stress.
  • Excellent observation, problem-solving, verbal and written communication skills.
  • Self-directed and able to work with minimal supervision.

Responsibilities

  • Present ANX Hospice Care services to patients and families.
  • Perform consultation and admission visits to assess patient needs.
  • Complete admission assessment and contractual agreement documentation.
  • Evaluate patients for hospice appropriateness through medical records review.
  • Document assessments, current medications, and initiate care plans.
  • Establish goals based on nursing diagnosis and incorporate patient and family in planning.
  • Assess patient’s clinical status and implement immediate interventions on admission day.
  • Coordinate medications and DME needs for same-day delivery.
  • Communicate admissions information and concerns in patient care meetings.
  • Perform routine and unscheduled nursing visits as required.
  • Educate patients and families about hospice care.
  • Participate in weekend on-call rotation, holidays, and after-hours as needed.
  • Perform comprehensive nursing evaluations and accept new admissions and emergency assignments.
  • Participate in QAPI program as required.
