Requirements

  • Must be able to work in East Bay or specific areas: Hayward, Union City, San Leandro, San Lorenzo, Castro Valley.
  • Current CA Registered Nurse (RN) license.
  • Current CPR card for Healthcare Providers.
  • 1 year of Registered Nurse (RN) experience in Hospice Care.
  • Licensed driver with an insured automobile in good working order.
  • Understanding of hospice philosophy and experience with terminally ill patients.
  • Demonstrated nursing skills and ability to cope with family emotional stress.
  • Excellent observation, problem-solving, verbal and written communication skills.
  • Self-directed and able to work with minimal supervision.

Responsibilities

  • Present ANX Hospice Care services to patients and families to facilitate admissions.
  • Perform consultation and admission visits to assess patient needs.
  • Complete admission assessment and contractual agreement documentation for hospice.
  • Evaluate patients for hospice appropriateness through medical records review.
  • Document assessments, current medications, and initiate appropriate care plans.
  • Write and initiate a plan of care that establishes goals based on nursing diagnosis.
  • Assess patient’s clinical status and implement immediate interventions on admission day.
  • Coordinate medications and DME needs for same-day delivery.
  • Communicate admissions information and concerns in patient care meetings.
  • Perform routine and unscheduled nursing visits as required.
  • Educate and address questions from patients and families regarding hospice care.
  • Participate in weekend on-call rotation, holidays, and after-hours as needed.
  • Conduct admissions at patients' homes, hospitals, or skilled nursing facilities.
  • Participate in QAPI program as required.
