About The Position

We provide innovative and compassionate services to individuals struggling with substance use and co-occurring disorders. We are looking for dedicated, caring and compassionate people to work in our Residential/Outpatient Treatment Facilities and Community Programs. As a HOPWA Case Manager, you will provide essential case management services for persons with HIV/AIDS and their families under the Housing Opportunities for Persons with AIDS (HOPWA) Program. This program is designed to prevent homelessness and improve the quality of life for those infected and affected by AIDS. Your role will involve interviewing prospective clients to determine their eligibility for services and identifying the specific services they require. You will be responsible for providing eligible clients with vouchers for essential services such as apartment security deposits, back rent (with eviction notice), utility bills (with shut-off notice), and food assistance. In addition to direct client services, you will facilitate client access to other community resources available to those with HIV/AIDS. It is crucial to document all client encounters and contacts made on behalf of clients, complete and submit billing documentation as appropriate, and maintain comprehensive client files. You will also be responsible for maintaining and reporting applicable statistics regarding programs and client services, as well as performing miscellaneous job-related duties as needed.
