About The Position

The Hospice Chaplain plays a vital role in providing spiritual and emotional support to patients and their families during end-of-life care. This position involves offering compassionate guidance, facilitating meaningful conversations about faith, hope, and grief, and helping individuals find peace and comfort in challenging times. The chaplain collaborates closely with interdisciplinary hospice teams to ensure holistic care that respects diverse beliefs and cultural backgrounds. They also provide crisis intervention, bereavement support, and assist in navigating ethical and spiritual dilemmas. Ultimately, the Hospice Chaplain contributes to enhancing the quality of life for patients and their loved ones by addressing spiritual needs with sensitivity and professionalism. As a Hopestone Hospice Chaplain, you will: Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes. Provide support for the spiritual needs of the patient/family. Attend interdisciplinary team meetings and participate in the patient care planning process. Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes. Meet mandatory continuing education requirements of the agency. Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements. Promote Agency philosophy and administrative policies to ensure quality of care. Demonstrate understanding of coordination and continuity of care by collaborating with the IDG. Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.

Requirements

  • One year of experience as a Chaplain in health care preferred
  • One or more years of Hospice experience required
  • CPR Certification
  • Valid Driver’s License
  • Valid Auto Insurance
