The HOPE Community Family Care Manager plays a crucial role in utilizing agency and community resources to assist residents in achieving lasting recovery from homelessness. This position is dedicated to promoting client self-sufficiency by providing clear pathways to housing, health, and financial outcomes. The Family Care Manager addresses individual needs related to transportation, employment, and medical/mental health issues, guiding clients through various application processes. Collaboration with community agencies is essential to secure additional resources and advocate against systemic barriers to homelessness. The role also involves supporting direct staff, the HOPE General Manager, and executive staff, fostering an environment of health, mutual respect, clear communication, and positive self-regard. In this position, the Family Care Manager will engage in case management with residents, which includes ongoing assessments, goal setting, crisis intervention, and referrals to necessary resources. The manager will collect vital information to identify each resident's housing and service needs, aiming to resolve immediate housing crises. This process involves refining and verifying the resident's housing and homeless history, barriers, goals, and preferences, which will be documented in a Needs Assessment within 14 days of admission. The development of Individualized Housing Plans will also occur within this timeframe, ensuring that residents receive the support they need to transition to permanent housing. The Family Care Manager will maintain regular engagement with residents, providing biweekly monitoring of housing goals until the household achieves permanent housing. Additionally, the role requires increasing residents' awareness of available services tailored to their individual needs and assisting them in accessing these resources to encourage personal responsibility and self-sufficiency. Maintaining accurate charts and records according to NASW standards, including the use of the Homeless Management Information System, is also a critical responsibility. The Family Care Manager will work closely with Resident Care Specialists to develop mutually beneficial relationships that support residents in reaching their housing, health, and financial goals.
