About The Position

The Homeless Outreach Worker (HOW) at CHOICE of NY plays a crucial role in assisting individuals living with mental illness and those experiencing homelessness. This position requires engaging with clients either on the streets or in shelters, providing advocacy, and facilitating connections to essential benefits and housing resources. The HOW is responsible for establishing a rapport with clients, setting appointments, and completing necessary paperwork in a timely manner. Upon assignment of a new client, the HOW must promptly contact the client to initiate the care process, ensuring that all required guidelines are followed. In this role, the Homeless Outreach Worker will set client goals and align care activities accordingly, monitoring progress and adjusting care plans as needed. Accurate and thorough documentation of activities and outcomes in case management systems is essential. The HOW will also plan and execute care activities to minimize potential billing and reimbursement issues for the agency. Effective time management is critical to achieving client outcomes within the designated time frames set by the supervisor. Regular communication with the Homeless Outreach Supervisor is necessary to address any emerging issues and make necessary adjustments. The HOW is expected to connect with each assigned client at least four times per month and review their caseload bi-monthly with the supervisor to ensure effective case management. Building productive relationships with key individuals at outside organizations is also a vital part of this role. The position involves conducting street outreach assessments and intakes, with a target of 10-15 intakes per week. The ideal candidate will demonstrate problem-solving skills, advocacy, and the ability to handle complex care coordination tasks.
