About The Position

- The Homeless Outreach Specialist at LIFESTREAM BEHAVIORAL CENTER is tasked with providing critical support and services to individuals experiencing homelessness. This role aims to engage with the homeless population to assess their needs, connect them with appropriate resources, and facilitate access to mental health and social services, ultimately working towards improving their quality of life and assisting them in achieving stable housing.

Requirements

  • High School Diploma or GED.
  • Minimum of 2 years of experience working in social services, specifically with homeless populations.
  • Proven track record in outreach and engagement with vulnerable communities.
  • Experience in case management and developing individualized service plans.
  • Familiarity with local community resources and services for homeless individuals.
  • Strong interpersonal and communication skills to effectively interact with diverse populations.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Excellent problem-solving skills and the ability to handle crisis situations calmly and effectively.
  • Proficient in Microsoft Office Suite and case management software.
  • Understanding of trauma-informed care practices and harm reduction strategies.
  • Ability to maintain confidentiality and adhere to ethical standards in social work.

Responsibilities

  • Conduct outreach activities to identify and engage with individuals experiencing homelessness in various settings, including streets, shelters, and community areas.
  • Assess the immediate needs of homeless individuals and provide appropriate interventions, including crisis management and basic necessities.
  • Develop and maintain relationships with local service providers, shelters, and community organizations to coordinate comprehensive care and resource referrals.
  • Facilitate access to mental health services, substance abuse treatment, and other supportive services to address the underlying causes of homelessness.
  • Assist clients in navigating social service systems, including housing applications, employment opportunities, and healthcare services.
  • Maintain accurate and up-to-date records of client interactions, assessments, and service plans.
  • Advocate for the needs and rights of homeless individuals within the community and with service providers.
  • Participate in community meetings, workshops, and training sessions to stay informed about best practices and resources for serving the homeless population.
  • Collaborate with a multidisciplinary team to develop and implement individualized service plans for clients.
  • Monitor and evaluate the progress of clients towards achieving stability and independence, adjusting plans as necessary to meet changing needs.
