About The Position
The Homeless Outreach Specialist is responsible for performing street-based outreach to engage and build rapport with unsheltered youth, individuals, and families experiencing homelessness. This role involves providing outreach and engagement services to identify appropriate interventions and linkages to permanent housing, shelter, health, mental health, and other supportive services. The specialist assists with housing navigation activities and document collection, and screens clients for homelessness prevention services.
