About The Position

The Homeless Outreach/Housing Navigator is responsible for identifying, assessing, and connecting individuals and families experiencing homelessness or at risk of homelessness with appropriate resources and services. This role requires strong interpersonal skills, a deep understanding of the local housing market and social service landscape, and a commitment to helping individuals achieve housing stability.

Requirements

  • High school degree
  • Peer Support Specialist Certificate
  • Strong interpersonal skills and the ability to build relationships with individuals from diverse backgrounds.
  • Knowledge of local housing resources and social service providers.
  • Experience with case management and crisis intervention.
  • Ability to work independently and as part of a team.
  • Excellent organizational and time management skills.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Bilingual language skills.
  • Minimum of 2 years of experience working with individuals experiencing homelessness or at risk of homelessness.
  • Knowledge of housing policies and regulations.
  • Certification in crisis intervention or mental health counseling.

Responsibilities

  • Actively seek out individuals and families experiencing homelessness or at risk of homelessness in the community.
  • Conduct needs assessments to identify barriers to housing and develop individualized service plans.
  • Build rapport with individuals and families to establish trust and foster long-term relationships.
  • Assist individuals and families in locating and securing affordable housing options.
  • Provide guidance on rental applications, lease agreements, and tenant rights.
  • Advocate for individuals and families with landlords and property managers to increase housing placement success.
  • Provide ongoing case management support to individuals and families after housing placement.
  • Connect individuals and families with essential services, such as mental health counseling, substance abuse treatment, and job training.
  • Monitor housing stability and address any challenges that may arise.
  • Collaborate with local agencies, organizations, and providers to coordinate services and resources for individuals and families.
  • Participate in community-based initiatives and advocacy efforts to address homelessness and related issues.
  • Collect and maintain accurate data on individuals and families served.
  • Prepare regular reports on program outcomes and service utilization.
