The RN Admission Nurse works under the supervision of the Nurse Manager or Assistant Nurse Manager. This role involves evaluating and admitting patients for all home care disciplines and making regularly scheduled visits as needed. Key duties include conducting admissions, revisits, OASIS data collection, developing the plan of care, referring ancillary services such as rehabilitation therapists, social workers, and home health aides, and completing required documentation. The RN Admission Nurse also assists with QAPI programs and performs other duties as assigned.

  • Performs admission visits and completes the Physician's Plan of Care in collaboration with the patient, family, physician, and other health care team members.
  • Obtains informed consent for treatment and informs patients of their rights and responsibilities.
  • Informs patients and families about available insurance coverage for Home Care services and non-reimbursable services.
  • Provides information regarding patients' rights to formulate Advance Directives.
  • Instructs patients and families on processes to follow for assistance after regular office hours.
  • Reviews general emergency preparedness instructions with patients and families.
  • Uses assessment skills and interviewing techniques to identify patient problems and care needs.
  • Completes OASIS assessment and collaborates with Coder and Intake Staff for correct ICD coding.
  • Formulates and initiates the plan of care including Home Care discharge planning.
  • Makes home visits to render professional nursing care and observes return demonstrations of health restoration/maintenance procedures.
  • Completes clinical records for required documentation in accordance with Home Care standards.
  • Serves as a liaison between Home Care and physicians, SOMC hospitals, other health care team members, and community agencies.
  • Communicates changes in patient status to patients, family members, caregivers, physicians, and other health care team members.
  • Participates in the 24-hour, 7 day a week on-call nursing coverage for patients/families as appropriate.
  • High School Diploma or successful completion of an equivalent High School Exam required.
  • Graduate from an accredited school of nursing required.
  • Licensed to practice as an RN in Ohio required.
  • Licensed to practice as an RN in Kentucky to be obtained after hire required.
  • BLS within 90 days of hire required.
  • Prior home care experience preferred.
  • BSN preferred.
