About The Position
The Home Health RN delivers compassionate, patient-centered nursing care in a home environment. This role focuses on helping patients recover, manage chronic conditions, and maintain independence while ensuring high-quality clinical outcomes.
Requirements
- Current RN license in good standing
- Degree from an accredited nursing program
- Valid CPR/BLS certification
- At least 1 year of nursing experience
- Excellent clinical judgment and problem-solving abilities
- Strong interpersonal and communication skills
- Ability to travel to patient homes and work independently

Responsibilities
- Conduct comprehensive in-home health assessments and ongoing evaluations
- Create and manage personalized care plans in collaboration with physicians
- Provide skilled nursing services including injections, IV therapy, catheter care, and wound management
- Track patient conditions and adjust care plans as needed
- Maintain detailed and timely clinical documentation
- Teach patients and caregivers about treatments, medications, and preventive care
- Communicate regularly with healthcare providers and interdisciplinary team members
- Follow infection control practices and ensure patient safety at all times
