About The Position

The Home Care Occupational Therapist provides direct occupational therapy services to patients in their homes. This role focuses on improving patients’ independence, safety, and ability to perform daily activities after illness, injury, surgery, or due to chronic conditions. The therapist works autonomously in the field while coordinating care with physicians and an interdisciplinary healthcare team.

Requirements

  • Degree in Occupational Therapy (BS, MS, or OTD) from an accredited program
  • Current and valid Occupational Therapist license in the state of practice
  • NBCOT certification (if required by state)
  • Valid driver’s license and reliable transportation for daily home visits

Nice To Haves

  • Clinical experience preferred
  • Home health experience strongly preferred
  • Familiarity with electronic medical records (EMR) systems preferred
  • Experience with geriatric, neurological, orthopedic, or post-acute populations is a plus

Responsibilities

  • Perform in-home occupational therapy evaluations to assess functional status and safety needs
  • Develop individualized, patient-centered treatment plans based on clinical findings
  • Deliver skilled therapy interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Train patients and caregivers in adaptive techniques, compensatory strategies, and safe transfer methods
  • Assess home environments and recommend modifications to improve safety and accessibility
  • Recommend, educate on, and assist with adaptive equipment and assistive devices
  • Document all evaluations, treatments, progress notes, and discharge summaries accurately and timely
  • Communicate and coordinate care with physicians, nurses, physical therapists, and other team members
  • Reassess patient progress and modify care plans as needed to meet goals
  • Educate patients and families on condition management, fall prevention, and functional maintenance
  • Ensure compliance with all agency policies, state regulations, and Medicare/home health guidelines
